There is an increase in the number of employees submitting fraudulent sick notes obtained from claiming to be doctors. There are several individuals claiming to be medical practitioners operating in our areas.              
               
The onus is on you to carefully check every document that is given to you by any doctor as a sick note and ensure that it contains the following information:              
·         The doctor’s name              
·         His/her qualifications              
·         Practice number              
·         Physical and Postal address              
·         Contact number              
·         Date of consultation              
·         Recommended period of sick leave              
               
Please remember that JJNIS does not accept sick notes that are clearly fraudulent and disciplinary action may be taken against those who submit them.
